A RECEIPT WILL BE ATTACHED TO THE ENVELOPE WITH THE REQUEST THAT THE SIGNATURE OF THE PILOT AND RECEIVING AGENT AT WFO SIGN THE RECEIPT OF THIS EVIDENCE. THE RECEIPT SHOULD BE RETURNED TO THE SEATTLE DIVISION.
