On 10/21/74, [redacted b6 b7C] O'Neill Investigations, Anchorage, Alaska, [redacted b6 b7C] of the SAC, advised that [redacted b6 b7C] was employed by [redacted b6 b7C] furnished an application prepared by [redacted b6 b7C] which lists his DOB as [redacted b6 b7C] At the time of his application in [redacted b6 b7C], he listed [redacted b6 b7C] He showed military [handwritten: St Louis] service with USAF, [redacted b6 b7C], with an honorable discharge. He served [redacted b6 b7C] operator while in the service.
As references he listed [redacted b6 b7C] [redacted b6 b7C], Boise, Idaho; [redacted b6 b7C] Pacific Northwest Bell Telephone, Kent Wash.; and [redacted b6 b7C] Nez Perce County Sheriff's Office, Lewiston, Idaho.
