A person reads every report. Corrections are made by hand, to the data — titles, dates, names, which pages belong together — never to the scans, and they can take a while to appear.

In compliance with letter, your Headquarters, regarding my retention in an active status in the Army Reserve, it is requested that the following indicated action be taken:
☐ Transfer to the inactive status list of the Standby Reserve. I presently hold primary MOS: _______________.
☐ Transfer to the inactive status list of the Standby Reserve upon recommendation of a Board of Officers.
☐ Transfer to Retired Reserve under paragraph _________, AR 140-160.
☒ Request that I be granted a waiver of the requirement for accruing the required retirement points during my last retirement year. The follow- ing condition(s) of hardship prevented my active participation in Reserve activities during that period. If waiver is granted, I will be able and willing to actively participate in Reserve activities in order to maintain an active status:

For the past three years since my separation from active duty my work has prevented my attending the necessary meetings. Last spring I requested and received a transfer of MOS from 6301 to 6101 per Special Order 90, your headquarters, 9 May 1955. I expected to be notified which meetings that I was supposed to attend, I never received such notification. However, I would like to become active in this military govern- ment unit. Due to my work as a Certified Public Accountant I may not be able to attend meetings regularly until after march. However, after that I would be happy to attend regularly.
