On 8/16/72, [REDACTED] Bureau of Vital [guess: Statistics], Toledo, Ohio, advised that her records reveal JAMES E. FUGATE, DOB 8/23/44, died Toledo, Ohio, on 7/8/71, as a result of injuries sustained in an aircraft accident while flying for Buckeye Air Service, Elyria, Ohio.
On 8/18/72, [REDACTED] Elyria, advised JAMES E. FUGATE, born 8/23/44, was a co-pilot [REDACTED] and was killed in Toledo aircraft accident in mid-1971.
