For the information of the file, investigation was initiated on 12/10/89 when a complainant telephonically contacted the San Diego Office furnishing information on captioned matter.
Information was obtained from the complainant, the complainant's husband, [REDACTED] and subsequently furnished telephonically to Seattle Case Agent [REDACTED]
Information was dictated on a timely basis to the Steno Pool; however, communications were misplaced and never submitted to Office of Origin.
In view of the Bureau guidelines on reporting information to Office of Origin, this matter was covered telephonically per conversations with Agent [REDACTED]
