On 5/5/77, Mr. DAMON ELDER, Administrator, Melwood Farm, Box 182, Olney, Maryland, telephone 924-5000, furnished the following information to SA CARLYLE R. THOMPSON:

ELDER advised that he is not familiar with an individual by the name JACK BRYAND COFFELT as receiving medical treatment at the Melwood Farm Alcoholic Treatment Center in Olney, Maryland, during 1971.

He recalls that in 1971 there was a Melwood Clinic located at 2029 Q Street, N. W., Washington, D. C., and advised that possibly this individual could have received treatment at that location.
