[REDACTED] was advised of the identities of the interviewing agents and purpose of the interview prior to his furnishing the following information:

[REDACTED] was previously employed by Northwest Orient Airlines, [REDACTED] [REDACTED], at Minneapolis, Minnesota, from 1966 through 1972, however during the month of June, 1971, [REDACTED] [REDACTED] during October, 1971, at St. John's Hospital, in Red Wing, Minnesota.
